29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. At the Amazon warehouse, Liverpool, NY, on or about 7/21/2021 and until 7/30/2021: Employees were manually lifting a conveyor bed section which was fourteen feet long and weighed 625 pounds. The conveyor bed section was set down on an employee's pinky finger resulting in an amputation. The employee had not been trained in the avoidance of unsafe conditions relative to this task.
