Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On 7/30/2021, employees of Antero Construction LLC were performing roofing work at 429 Fuller Drive, Wentzville, Missouri. Employees were working on a residential roof approximately 19 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system while on the roof.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On 7/30/2021, employees of Antero Construction LLC were performing roofing work on a residential building located at 429 Fuller Drive, Wentzville, Missouri. A Gorilla Multi-Position Ladder was placed on the West side of the house and used to access the upper landing surface of the roof. The side rails of the ladder did not extended 3 feet above the upper surface.
