On or about July 28, 2021, and at times prior thereto, at the powder coating booth, the employer did not install automatic sprinklers, or other approved automatic extinguishing equipment, to the interior areas of each powder spray booth.  This exposed employees to fires.

29 CFR  1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.  On or July 29, 2021, and at time prior thereto, at the facility, the employer did not provide a medical evaluation for an employee that was in use of a respirator.  This exposed the employee to health hazards.

29 CFR  1910.134(g)(1)(i)(A):  Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function.  On or July 28, 2021, and at time prior thereto, at the powder coating area, the employer did not ensure an employee, who is required to wear a tight-fitting respirator, did not have facial hair that came between the sealing surface of the facepiece and the face.  This exposed the employee chemical hazards.

On or about July 29, 2021, and at times prior thereto, at the blasting areas, the employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes completed a tag and posted it at the sorbent bed or at the compressor. On or about July 28, 2021, and at times prior thereto, at the establishment the employer did not develop and document step-by-step lock-out/tag-out procedures for the control of potentially hazardous energy of specific equipment when employees are required to perform Lock-out/Tag-out of equipment.  This exposed the employees to the hazards of an unexpected release of energy.
