29 CFR  1910.36(d)(1):Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  a) Main Entrance to Building - On or prior to July 29, 2021,  exit doors for the main entrance to the building were found to be manually locked at approximately 4:30pm every night and then unlocked in the morning at approximately 8:30am every day.  Employees needed a key to unlock the exit doors in order to exit the building after the exit doors were manually locked at approximately 4:30pm.  b) Side Entrance to Building - On or prior to July 29, 2021,  the exit door for the side entrance to the building was found to be manually locked at approximately 4:30pm every night and then unlocked in the morning at approximately 8:30am every day.  Employees needed a key to unlock the exit door in order to exit the building after the exit door was manually locked at approximately 4:30pm.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Doorway Next to Copier -  On or prior to July 30, 2021, relocatable power taps (RPTs) or power strips were not installed and used in accordance with instruction in the listing, labeling or certification such as, but not limited to, daisy chaining two RPTs (POWER TAP 31F8 and a ESP RPT, S/M: 420491)  together that were powering a Copier (Konica Minolta, Inc., Model FK-514), a ring door bell, cell phone and water cooler.  b) Editorial Room - On or prior to July 30, 2021, relocatable power taps (RPTs) or power strips were not installed and used in accordance with instruction in the listing, labeling or certification such as, but not limited to, daisy chaining RPTs together by using 3 outlet connectors to power computer equipment, televisions, and a GE portable A/C unit.    c) Office Room - On or prior to July 30, 2021, relocatable power tap (RPT) or power strip was not installed and used in accordance with instruction in the listing, labeling or certification such as, but not limited to, powering a portable Black + Decker A/C unit and  computer equipment.
