29 CFR 1910.178(l)(1): The employer did not ensure that each power industrial truck operator was competent to operate a power industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1): a) U.S. Postal Service dba Music City Annex, Outside dock/warehouse floor: On or about August 2, 2021, and at times prior, the employer exposed an employee to a struck-by/crushing hazard, in that Automatic Ground Vehicle training/evaluation were not provided to employees while operating AGVs on the USPS warehouse floor.
