Located at Building 8, 46-66 Green Mill in Blacklick, Ohio: On or about August 2, 2021, the employer failed to maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed roofing work.

Located at Building 8, 46-66 Green Mill in Blacklick, Ohio: On or about August 2, 2021, employees were operating pneumatic nailers without eye protection, thereby exposed to struck by hazards.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. Located at Building 8, 46-66 Green Mill in Blacklick, Ohio: On or about August 2, 2021, employees were performing roofing work from a steep pitched two story building without fall protection, thereby exposed to fall hazards from approximately twenty foot.  Gueros Roofing, LLC was previously cited for a willful violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1275907, citation number 1, item number 1, and was affirmed as a final order on 02/28/2018, with respect to a workplace located at 4900 North Arbor Woods, Cincinnati, Ohio 45248.  Gueros Roofing, LLC was previously cited for a repeat violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1257062, citation number 1, item number 1, and was affirmed as a final order on 09/28/2017, with respect to a workplace located at 5070-5072 Buffalo Run, Westerville, Ohio 43081.  Gueros Roofing, LLC. was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1219529, citation number 1, item number 1, and was affirmed as a final order on 05/30/2017, with respect to a workplace located at 602-618 Vista Drive, Gahanna, Ohio 43230.
