Instance A) Southwest part of the main shop, employees utilized a band saw (HEM Saw/SN-621298) without a point of operation guard. Instance B) Northeast side of the fabrication shop, employees utilized an American Lathe (no S/N) without a chuck guard. Instance C) West side of the fabrication shop, employees utilized an ACRA High-Speed Lathe Id #FEL-1640GNX without an adequate chuck guard. Instance D) CH-1297, Mori Seiki High-speed lathe (No identifying marks).

On or about August 2, 2021, on the east side of the main bay, employees utilized an electrical extension cord that had strain relief damage in order to power tools, fans, and other equipment while performing machining operations.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to an outer jacket or insulation) and for evidence of possible internal damage, such as a pinched or crushed outer jacket. On or about August 2, 2021, on the east side of the main bay, the employer failed to prevent employees from using an electrical extension cord that had strain relief damage and used to power tools, fans, and other equipment while performing machining operations.
