29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  New residential construction located on Lot #31 at 140 Hollis Street, Fayetteville, GA 30214, Second level and second level top plate: On or about 07/26/2021, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees engaged in truss installation activities.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) New residential construction located on Lot #31 at 140 Hollis Street, Fayetteville, GA 30214, Second level top plate: On or about 07/26/2021, the employer exposed employees to fall hazards of 8-feet, 9-inches to the lower level, in that a fall protection system was not provided to employees installing roof trusses from the top plate.  b) New residential construction located on Lot #31 at 140 Hollis Street, Fayetteville, GA 30214, Second floor: On or about 07/26/2021, the employer exposed employees to fall hazards of approximately 10-feet to the lower level, in that a fall protection system was not provided to employees engaged in roof truss installation. Employees were wearing full body harnesses but were not tied off to a life-line or lanyard.

29 CFR  1926.1051(a):Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  New residential construction located on Lot #31 at 140 Hollis Street, Fayetteville, GA 30214, Stairwell on ground level: On or about 07/26/2021, the employer exposed employees to falls up to 10-feet, 2-inches to the lower concrete slab, in that safe access to the second level was not provided to employees as wood 2x4 cleats nailed to the framing were used as a means of access. The 2x 4 cleats nailed to the framing being used like a ladder had spaces of 20-inches between each 2x4 which were used as an alternative to a ladder or stairway.
