29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment or ensure inspections were made by competent persons designated by the employer:   (a)Diamond Roofing Corporation at 525 Abraham Court in Rapid City, SD 57703: On or about and at times prior to July 28, 2021,  the exposing employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Employees were exposed to fall hazards while conducting roofing operations and eye hazards associated with the use of a pneumatic nail gun without eye protection. This condition exposed employees to eye hazards and fall hazards greater than 10 feet.

29 CFR  1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  (a)Diamond Roof Corporation at 525 Abraham Court in Rapid City, SD 57703: On or about and at times prior to July 28, 2021, the exposing employer did not ensure employees were protected from eye hazards. Employees were not utilizing appropriate eye protection when operating pneumatic nail guns. This condition exposed employees to eye hazards.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)Diamond Roofing Corporation at 525 Abraham Court in Rapid City, SD 57703: On or about and at times prior to July 28, 2021, the exposing employer did not ensure an employee was protected from fall hazards. An employee was performing roofing work on a single-family home and did not utilize an appropriate fall protection system. This condition exposed the employee to a fall hazards greater than 10 feet.
