An employee, engaged in residential construction activities operated a Lull, Model 1044C-54, Series II, S/N: 0160032467 without being provided operator training, an observation, and certification to operate the equipment.   Violation occurred on or about August 16, 2021

Employees engaged in residential framing activities worked at the outside edge of the roof while installing a dormer, at heights greater than 6 feet, and were not protected from falls by use of a personal fall arrest system or guardrail system.
