The ends of a platform greater than 10 feet in length extended over its supports more than 18 inches and the platform was not designed and installed so that the cantilevered portions of the platform were able to support employees without tipping, or had guardrails which blocked employee access to the cantilevered end. Location: 733 Westminster Rd, Brooklyn, NY. On or about 7/28/2021 a) An employee worked from a cantilever section of a ladder jack scaffold platform and it was extended more than 80 inches from its support.
