29 CFR  1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident   On or about 08-03-2021 at 10:30 AM, located at the Pentagon, 1155 Defense Pentagon, Arlington, VA 20301, on the bus platform, a Pentagon Force Protection officer/employee was fatally stabbed by an assailant.  The event occurred on 08-03-21 at 10:30 AM and the employer did not report the work-related fatality to OSHA until 08-04-21 at 7:04 AM.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
