29 CFR 1926.416(a)(1):No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means. Job site: 2121 Robinson Blvd, Pittsburgh, PA: On or about August 3, 2021, employees were working to remove a 480V electrical cable from a panel box and did not de-energize the circuit; one employee and company foreman received arc flash burns as a result of attempting to relocate the cable without it being de-energized.

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: (a) Job site: 2121 Robinson Blvd, Pittsburgh, PA: On or about August 3, 2021, employees were not protected from electrical shock hazards in that unused openings in the 240V electrical panel box feeding from the transformer were not covered, thus exposing employees to live electrical parts.
