29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing injuries from overturning forklifts while operating them without using operator restraint systems (seatbelts):  Location - 13 Centennial Dr., Peabody, MA 01960 - Dock 7.  Instance 1: On or about 8/3/2021, Toyota Electric Forklift Truck, model number 8FBCU25, serial number 68097, was operated without the use of the seatbelt, in that the seatbelt was not fastened while an employee was operating the forklift.  Instance 2: On or about 8/3/2021, a Toyota Electric Forklift Truck, model number 8FBCU25, serial number 68069, was operated without the use of the seatbelt, in that the seat did not have a seatbelt installed on the seat, as required by the manufacturer.  Not using a seatbelt exposes the forklift operator to crushing injuries in the event the forklift overturned.
