29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: On or about August 4, 2021, employees were exposed to fall hazards in excess of 10 feet, while using a stairway to access elevated work platforms. The stairway did not have a stairrail system along each unprotected side or edge, on a commercial construction site in Catlin, Illinois.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: On or about July 21, 2021, employees were exposed to fall hazards in excess of 10 feet, while using an extension ladder to access elevated work platforms. The extension ladder had structural defects, and the employer did not tag and remove the extension ladder from service, on a commercial construction site in Catlin, Illinois.
