29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which where subjected to heat, might produce flammable or toxic vapors: a) At the facility, on or about July 23, 2021 - A hydraulic tank on the vehicle was not cleaned so thoroughly as to make absolutely certain there were no flammable materials present. An employee received 2nd and 3rd degree burns on the arm and neck when welding on a hydraulic tank resulting from a flash that occurred.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) At the facility, on or about July 23, 2021 - An in-patient hospitalization of an employee was not reported to OSHA within twenty-four hours. An employee was hospitalized with 2nd and 3rd degree burns on July 23, 2021 and the employer did not report the in-patient hospitalization to OSHA until July 28, 2021.
