29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) Jobsite: On or about 7/30/21 the employer did not ensure that each employee, installing sheathing on a new residential apartment complex, had attached their harness to the self-retractable lifeline while conducting work from the aerial lift while working 18 feet from the ground.

29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:   (a) Jobsite: On or about 7/30/21, the employer did not ensure that employees inspected their self-retracting lanyards for defects prior to starting work; in that they were using self-retracting lifelines which had a damaged label and a self-retracting load indicator that was covered with electrical tape and the label was not visible.
