The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 5, 2021, employees were exposed to a fall of approximately 10.5 feet while performing roofing activities on a residential home located at 14174 Whitney, Strongsville, Ohio 44136. The employees were working on all areas of the roof and the employer did not ensure that they were trained on how to recognize and minimize fall hazards.
