29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about August 5, 2021 and at times prior thereto, two employees were repairing a leak without attaching the lanyard to the anchor point on the wearer's back. Employees were working approximately 25 feet from the ground, exposing the employees to a fall hazard.

On or about August 5, 2021, two employees were using a saw to cut tiles without wearing appropriate eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.300(b)(1):When power operated tools are designed to accommodate guards, they were not equipped with such guards when in use. On or about August 5, 2021, two employee were cutting roof tiles with a 12 inch saw that did not have the guard attached, exposing employees to the hazard of being struck-by the rotating blade.
