29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as clearing jams, setting up machines.  a) BMC Truss 7239 Expressway, Missoula MT 59808: On or about August 5, 2021, and at times prior, employees were exposed to amputation hazards due to the unexpected energization of equipment while performing maintenance on the unguarded chain and sprocket to adjusting for proper tensioning on the ALS Machine.
