On July 18, 2021 at 2901 Sherman Street in Wausau, Wisconsin, an employee was using a company designed and made Beam Separator hook device for rigging an I-beam to an overhead crane without prior equipment design safety certification and approval.

29 CFR 1910.179(n)(3)(i): Handling the load. Moving the load. The load shall be well secured and properly balanced in the sling or lifting device before it is lifted more than a few inches. On July 18, 2021 at 2901 Sherman Street in Wausau, Wisconsin, an employee was lifting an I-beam approximately four inches with an overhead crane when the beam separator hook device released from the end of the I-beam dropping it.
