29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about August 5, 2021 and at times prior thereto, two workers were repairing a leak without attaching the lanyard to the anchor point on the wearer's back. Workers were working approximately 25 feet from the ground, exposing the workers to a fall hazard.
