29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   a) On August 5, 2021, an employee was using upper deck of tubular weld scaffold that was not fully planked. The planks did not come up to the guardrails exposing the employees to fall hazards of approximately 14 feet high.  b) On August 5, 2021, an employee was using an A-frame horse scaffold with a single plank, approximately 3 to 4 feet above the ground.  NOTE: Certification of corrective action is required for this item.
