29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  A) 37 Redmon Rd Flemington, NJ 08077   Two employees engaged in sheathing activities using a pneumatic nail gun and a Makita circular saw were exposed to eye injury hazards as a means of eye protection was not utilized.   Hazard noted on or about 08/06/21  Elite 5 Contractors Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1506080, citation number 1, item number 1 and was affirmed as a final order on 2/12/21, with respect to a workplace located at Lot 21 E Crescent Ave & Roxbury Road Mahwah, NJ 07430.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  A)31-37 Redmon Rd Flemington, NJ 08077  Three employees were exposed to fall hazards on two separate instances on the same jobsite. Two employees were exposed to a fall hazard of 10'10'' while performing sheathing work. Another employee was exposed to a fall hazard of 9'6'' while performing framing activities. All three employees had no means of fall protection in place.  Hazard noted on or about 08/06/21  Elite 5 Contractors Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1506080, citation number 1, item number 2 and was affirmed as a final order on 2/12/21, with respect to a workplace located at Lot 21 E Crescent Ave & Roxbury Road Mahwah, NJ 07430.
