29 CFR  1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Jobsite:  The employer did not ensure that employees who were exposed to a fall hazard when installing asphalt shingles on the roof were protected by the use of guardrails, safety nets or personal fall arrest systems.  On or about 27 March 2021, an employee sustained serious injuries when he fell from the roof while in the process of re-roofing a single-family residential building

Jobsite: The owner did not maintain a written certification of fall protection training for the employee that was exposed to fall hazards when installing the new roof.
