29 CFR  1926.451(a)(6):Scaffolds were not designed by a qualified person:  (a) At a worksite located at 1426 Georgetown Dr. Less Summit, MO, an employee engaged in a window installation was exposed to fall hazards when working on top of a wood constructed scaffold greater than 10 feet without fall protection.  The scaffold was built by wood by  a person that was not qualified on construction of a scaffold. This was documented on or around 9 Aug 2021.

29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  (a) At a worksite located at 1426 Georgetown Dr. Less Summit, MO, an employee engaged in a window installation was exposed to fall hazards when working on top of a wood constructed scaffold greater than 10 feet without fall protection.  The scaffold consisted of one horizontal wood plank that was not fully planked or decked. This was documented on or around 9 Aug 2021.

29 CFR  1926.451(b)(2):Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  (a) At a worksite located at 1426 Georgetown Dr. Less Summit, MO, an employee engaged in a window installation was exposed to fall hazards when working on top of a wood constructed scaffold greater than 10 feet without fall protection. The scaffold's walkways were constructed of lumber found on the jobsite.  Walkways on the scaffold were found to be less than 18 inches wide. This was documented on or around 9 Aug 2021.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) At a worksite located at 1426 Georgetown Dr. Less Summit, MO, an employee engaged in a window installation was exposed to fall hazards when accessing a wood constructed scaffold from a ladder, greater than 10 feet.  An extension ladder was used to access the scaffold but the ladder did not extend at least 3 feet above the scaffold surface being accessed. This was documented on or around 9 Aug 2021.
