29 CFR  1904.7(b)(5):  If the injury or illness did not involve death, one or more days away from work, one or more days of restricted work, or one or more days of job transfer, the employer did not enter a check mark in the box for cases where the employee received medical treatment but remained at work and was not transferred or restricted:  A.)  Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about June 22, 2021 and times prior thereto:  The employer did not classify Case No. 1 on the 2021 OSHA 300 log as "Other recordable case" in column J for an incident that occurred on June 22, 2021.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  A.)  Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about June 22, 2021 and times prior thereto:  Column F for Case No. 1 on the 2021 OSHA 300 log provides the injury and the part of the body affected, but does not provide the object that directly injured the employee.    B.)  Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about June 22, 2021 and times prior thereto:  Column F for Case No. 2 on the 2021 OSHA 300 log provides the injury and the part of the body affected, but does not provide the object that directly injured the employee.  C.)  Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about July 20, 2021 and times prior thereto:  Column F for Case No. 3 on the 2021 OSHA 300 log provides the part of the body affected, but does not provide a description of the injury or the object that directly injured the employee.
