Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about June 22, 2021 and times prior thereto: Column F for Case No. 1 on the 2021 OSHA 300 log provides the injury and the part of the body affected, but does not provide the object that directly injured the employee. B.) Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about June 22, 2021 and times prior thereto: Column F for Case No. 2 on the 2021 OSHA 300 log provides the injury and the part of the body affected, but does not provide the object that directly injured the employee. C.) Zenith Systems, LLC DBA Keystone Electric Construction Company, on or about July 20, 2021 and times prior thereto: Column F for Case No. 3 on the 2021 OSHA 300 log provides the part of the body affected, but does not provide a description of the injury or the object that directly injured the employee.
