OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:   (a)  On or about August 10, 2021, and at times prior, the employer did not ensure that employees were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:     (1)  Rotary Lift, model SPOA10N8T0, serial #DKR19B0079, did not have the locking mechanism locked  into place while the lift was raised and in use.     (2)  Rotary Lift, model SPOA10N8G0, serial #DJQ21C0195, did not have the locking mechanism locked into place while the lift was raised and in use.     (3)  Auto lifts were not inspected periodically by a qualified inspector at least annually to ensure that they were properly used, maintained and functioning.  Abatement Note:  Among other methods, feasible and acceptable methods of abatement would be to follow the requirements of ANSI/ALI ALOIM - Standard for Automotive Lifts - Safety Requirements for operation, Inspection and Maintenance, which includes, among other requirements, the following:    1.  Ensure that operators use all applicable safety features provided on the automotive lifts, which includes utilizing locking mechanisms. 2.  Institute a training program for the employees on the safe use and operation of automotive lifts and their safety features.  3.  Ensure that all automotive lifts are inspected by a qualified lift inspector at least annually.
