29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   At a worksite located at W966 County Road ZZ, Kaukauna, WI 54130; the employer did not protect an employee installing steel roofing material on a residential garage from fall hazards with a guardrail system, safety net system or personal fall arrest system. The employee was exposed to an approximate 10 foot fall at the eve edge and 17 foot fall from the peak of the residential garage.

29 CFR  1926.503(b)(1):  The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training:   At a worksite located at W966 County Road ZZ, Kaukauna, WI 54130; the employer did not verify through written certification record that employees were provided training regarding fall hazards and use of fall protection equipment when an employee was installing steel roofing material on a residential garage and was exposed to fall hazards.  All provisions of 29 CFR 1926.503(a)(2)(i) through (vii) must be contained in a training program for each employee exposed to fall hazards. Key elements of training include, but are not limited to the following:  1) Fall hazards in the work area 2) Procedures for installing, inspecting and maintaining fall protection equipment 3) Use and maintenance of fall protection equipment
