29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. (a) Location - 36 Follen Street, Cambridge, MA On or about 8/10/21, employees were exposed to electrical shock hazards while using a flexible electrical cord that had the outer sheathing pulled away from the strain relief device, exposing the insulated conductors.
