29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. a. On or about August 11, 2021 at the Sutherlands Lumber store located at 2590 Clime Road in Columbus, Ohio, the emergency exit in the South East corner of the store was secured by a 2"x6"x8' board wedged between the panic bar and frame of the door.

29 CFR 1910.178(l)(1)(ii):Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). a. On or about August 11, 2021 at the Sutherlands Lumber store located at 2590 Clime Road in Columbus, Ohio, the employer permitted employees to operate Powered Industrial Vehicles without being properly trained exposing employees to struck by hazards.

29 CFR 1910.213(h)(1):The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will deflect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed. a. On or about August 11, 2021 at the Sutherlands Lumber store located at 2590 Clime Road in Columbus, Ohio, the lower exposed portion of the blade of the counter top saw did not have guards that automatically adjusted itself to the thickness of the stock exposing employees to safety hazards of an unguarded saw blade. b. On or about August 11, 2021 at the Sutherlands Lumber store located at 2590 Clime Road in Columbus, Ohio, the lower exposed portion of the blade of the radial arm saw at building 9 did not have guards that automatically adjusted itself to the thickness of the stock exposing employees to safety hazards of an unguarded saw blade.
