29 CFR  1910.178(m)(5)(ii):A powered industrial truck is unattended when the operator is 25 ft. or more away from the vehicle which remains in his view, or whenever the operator leaves the vehicle and it is not in his view.  On or about August 11, 2021 and times prior thereto , the employer did not ensure powered industrial trucks were not being left unattended and powered ON by either distance or operator field of view in the following instances:  a) PIT out of the field of view of operator near the IC Department Desk b) PIT unattended by distance near Dock 127 c) PIT unattended by distance near Dock 22 d) PIT unattended by distance near Aisle 603 e) PIT unattended by field of view of operator near Replenishment Department Desk f)  PIT unattended by field of view of operator near Dock 150 g) PIT unattended by distance near Aisle 508

29 CFR  1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  On or about August 11, 2021 and times prior there to, the employer did not ensure that portable fire extinguisher locations were clearly and visibly identified in order to be readily accessible during an emergency in the following instances:  a) Warehouse entry door adjacent to the Human Resources Office, 1st floor.  b) Designated Emergency Exit Door located adjacent to Dock 124. c) Designated Emergency Exit Door located adjacent to Dock 120. d) Located on the left and right sides of the bay door to the Battery Charging/Floor Scrubber Storage Room
