29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Worksite: The employer did not ensure that employees who were required to work around buried energized power lines were protected against electric shock and arc flash burns by de-energizing the circuit and grounding it. On or about August 10, 2021, an employee sustained burns to his hands as a result of an arc flash while drilling into an energized underground high voltage electrical line.

29 CFR 1926.416(a)(2): In work areas where the exact location of underground electric power lines was unknown, employees using jackhammers, bars, or other hand tools which may contact a line were not provided with insulated protective gloves. Worksite: The employer did not ensure that the employee who was required to work on an underground energized high voltage power line was provided with insulated protective gloves. On or about August 10, 2021, an employee sustained burns to his hands as a result of an arc flash while drilling into an energized underground high voltage electrical line.
