Staten Island, NY - On or about on July 21, 2021, the employer issued a notice of separation to a carrier because the employee reported a work-related injury. c) 3301 Veterans Rd. West Staten Island, NY - On or about on July 15, 2021, the employer issued a notice of separation to carrier because the employee reported a work-related injury. Abatement certification s required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
