On or about August 11, 2021, during the construction of guardrails for a roofing project at Boulevard Elementary School located at 1749 Lee Rd, Cleveland Heights, OH 44118; the employer did not ensure the supervisor on site had received additional training on the skills, knowledge, and ability to recognize inadequacies and the compliance construction of the guardrail system used. In that, the guardrail system built on-site was missing the mid-rail sections to complete the guardrail system.
