29 CFR 1910.178(l)(4)(ii)(B):Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: On or about August 8, 2021 a forklift operator operated a sit down electric forklift on wet flooring and ran into a racking system in Row 7 and 8 and the employer did not ensure that the employee was retrained prior to operating the fork truck after the accident.
