29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 77 Paris Ave., Worcester, MA: On or about August 12 2021, employees were observed using ladders that did not extend 3 feet past the upper landing surface of the roof they were accessing. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19 (d) 1.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: a) 77 Paris Ave., Worcester, MA: On or about August 12 2021 , employees were observed performing roofing work near the edge of a roof more than twenty feet above the ground below and they were not protected by guardrail systems, safety net system, or personal fall arrest system. JRO Roofing, Inc was previously cited for a violation of this occupational safety 29CFR1926.501 (b) 13, which was contained in OSHA inspection number 1495419, citation number 1 item number 2a and was affirmed as a final order on 11/23/2020, with respect to a workplace located at 346 Grafton St., Worcester. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19 (d) 1.
