On or about 8/17/2021, a ladder used to aid in removing mortar from a stone archway was used on unstable ground, and exposed employees to fall hazards.

Location - 1025 Lawrence Street Lowell, MA 01852 - Lowell Cemetery - Entrance. On or about 8/17/2021, the employer did not implement engineering controls listed in Table 1, section (vii) for a Bosch 11255VSR Bulldog Extreme 1 in. Rotary Hammer that had been used to remove mortar containing silica from a granite structure.
