Honey Brook, PA: (a) On or about August 17, 2021, the employer did not provide effective hazardous chemical training to employees engaged in cutting and grinding activities on stainless steel materials, such as but not limited to:  - Hot Rolled Carbon Steel which contains metals such as, but not limited to chromium, nickel, iron, etc.  - 304/304L Stainless Steel which contains metal such as, but not limited to chromium, nickel, iron, manganese, etc. - 316/316L Stainless Steel which contains metal such as, but not limited to chromium, nickel, iron, manganese, molybdenum, etc. - Aluminum Alloys which contain metals such as, but not limited to aluminum, copper, cobalt, lead, tin, etc.  (b) On or about August 17, 2021, the employer did not provide effective hazardous chemical training to employees using propane forklifts inside the facility, exposing employees to propane gas and carbon monoxide.
