Maintenance employees regularly performed servicing and maintenance on equipment that required the use of lockout/tagout. This condition exposed employees to amputation hazards and hazardous energy. Serta Simmons Bedding, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1171238, Citation 1 Item 1, and was affirmed as a final order on March 8, 2017, with respect to a workplace located at 601 Gateway Blvd, Monroe, OH 45050.
