29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water: a) Port of Anchorage POL-1 POL- 2, East (1A) and Mid-Section bollard and dolphins: On or about August 16, 2022, employees accessed the gangway for snow/ice removal, longshore operations and minor maintenance with unguarded sections where the gangway met the landings, bollards, and or mooring dolphins, exposing employees to fall hazards into the water.

29 CFR 1917.112(c)(3):The top surface of guardrails installed before October 3, 1983, were not at least 36 inches high.: a) Port of Anchorage POL-1 Gangway: On or about August 16, 2021 employees used the gangway for snow/ice removal and longshore operations, the guardrail measured approximately 31 to 33 3/4 inches, exposing employees to fall hazards into the water below.
