Employees are using extension cords that have damaged sheathing that generate electricity to their power tools that were being used on the job site.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. The employer is failing to protect employees from fall hazards. This was most recently documented on or about August 16th, 2021 at 8017 Austin Street, Omaha, Nebraska. Employees were observed working on a residential structure engaged in framing operations. Employees were standing on top of the trusses/top plates without any fall protecting. Employees were working in excess of 8 feet - The measurements were taken inside the residential structure from the bottom plate of the framing to the top plate of the framing.

The employer is failing to protect employees from fall hazards. This was most recently documented on or about August 16th, 2021 in Omaha, Nebraska. The stairwell was used by two employees to egress from the 3rd floor landing area to the 2nd floor and it was not equipped with a stair rail system along each protected side or edge - the measurement from the lower level to the top landing area of the 3rd floor was over 9 feet.
