29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced:  (a)  TIRE WAREHOUSE, REAR OF FACILITY - Employees used an 11 foot high mobile ladder stand to access the racking where tires were stored, and the employer did not remove the ladder stand from service when the bottom step of the ladder stand was broken and disconnected from the frame of the ladder, on or about August 11, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.303(b)(8)(i): Electrical equipment was not firmly secured to the surface on which it was mounted:  (a)  REPAIR GARAGE NEAR LIFT FOUR - Employees performed automotive repair work in the repair garage where a 110 volt two receptacle outlet box that is designed to be secured was not mounted to a surface and was dangling on the end of a piece of conduit, on or about August 11, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  (a)  TIRE WAREHOUSE, BACK OF BUILDING, NEAR EXIT DOOR - Employees operated the electric light contactor switch mounted on the wall in the warehouse, and the electrical box where the switch was mounted was not equipped with a front cover which exposed employees to the energized electrical conductors, on or about August  11, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  FACILITY WIDE - Employees performed automotive repair duties where they used multiple products including but not limited to Pyroil Brake Parts Cleaner, Acetone, and Acetylene, and the employer had not provided any training to the employee(s) on the hazards of the chemicals that they were using, or on utilizing safety data sheets and/or pictograms, on or about August 11, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
