29 CFR 1910.29(b)(15): The employer did not ensure that guardrails met the requirement that manila or synthetic rope used for top rails or midrails were inspected as necessary to ensure that the rope continued to meet the strength requirements in paragraphs (b)(3) and (5) of this section. On or about August 10, 2021, the synthetic webbed feed gate (V-gate) on the rig floor was damaged. The nylon was broken and cut in several sections. This condition exposed employees to fall hazards.

29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about August 10, 2021, at Rig #26, an electrical 600V nominal cord had insulation damage. Employees were exposed to electrical burns and / or electrical shock hazards.
