29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   The employer is failing to protect employees from fall hazards associated with roofing operations  where the ground to the top of the eve was greater than 6 ft. The most recent occurrence of this was evident during the inspection of the job site located at 8507 Concord Street, Omaha, NE 68101 - The employer had 3 employees standing on the roof without using any fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Employer is failing to protect employees from fall hazards associated with roofing operations on new residential home located at 8507 Concord Street, Omaha, NE 68101;  An extension ladder was used for access and egress to the roof and it did not extend at least 3 feet above the surface being accessed.  Also, the ladder was also not secured at its top to a rigid support.
