Employer is failing to protect employees from fall hazards associated with roofing operations on new residential home located at 8507 Concord Street, Omaha, NE 68101; An extension ladder was used for access and egress to the roof and it did not extend at least 3 feet above the surface being accessed. Also, the ladder was also not secured at its top to a rigid support.
