29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems were inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components were removed from service. On or about August 10, 2021, on the rig floor; the employer did not ensure that personal fall protection systems were inspected before initial use during each work shift. The spring loaded easy rider clamps that are used as part of the ladder safety system were defective. This exposed employees to fall hazards.
