29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  The employer is failing to protect employees from fall hazards when on a scaffold. This was most recently documented on or about August 17, 2021 at a job site located at 20916 Barbara Plaza Cir., Elkhorn Nebraska.  Employees were utilizing a tubular framed scaffold, which was not fully decked or planked at all working levels, thus exposing the employees to potential fall hazards.

29 CFR  1926.451(f)(3):Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  The employer is failing to protect employees from fall hazards associated with utilizing inadequate scaffolding. This was most recently documented on or about August 17, 2021 at a site located at 20916 Barbara Plaza Cir., Elkhorn Nebraska.  Employees were using a tubular framed scaffold that had not been inspected by a competent person for visible defects.

29 CFR  1926.452(c)(2):Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally, and automatically square and align the vertical members so that the erected scaffold is always plumb, level, and square.  The employer is failing to protect employees from fall hazards when on a scaffold. This was most recently documented on or about August 17, 2021 at a job site located at 20916 Barbara Plaza Cir., Elkhorn Nebraska.  Employees were utilizing a tubular framed scaffold, that did not have the frames fully braced at all working levels, thus exposing the employees to potential fall hazards.

29 CFR  1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   The employer is failing to protect employees from fall hazards when performing painting activities on a residential structure. This was most recently documented on or about August 17, 2021 at a job site located at 20916 Barbara Plaza Cir., Elkhorn Nebraska.  Two employees were working on the roof of a residential structure performing painting activities without fall protection at heights of over 6 feet and not utilizing any form of fall protection.
