29 CFR 1910.304(g)(5):The path to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective: On or about August 11, 2021, support equipment was not grounded in accordance with NEC (National Electrical Code) Article 250 in the following instances: 1) Generator # G-74, Whisper-Watt generator, Model #DAC-150SSJU3, Serial #7600815 next to the Company man's trailer was not grounded. 2) Jefferson Electric General Transformer, 1121/2 KVA 480 Volt to 120/208 Volt in front of the Tool pusher's trailer was not grounded.

29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about August 11, 2021, Stoneham Rig #6 location, the Directional Driller's trailer had two staircases that were not equipped with a mid-rail opening that measured at least 19 inches. This exposed employees to fall hazards.
