29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) Ralph L. Wadsworth Construction Company, LLC at 1097 S Parker Rd, Denver, CO 80231: On or before August 18, 2021, the controlling and exposing employer did not ensure that an employee working next to a near-vertical wall of an excavation that was approximately 12 feet in depth was protected by an adequate protective system. An employee was working at the bottom of the excavation near the wall while a metal plate was being installed vertically by a back hoe which was located on the street above. A section of the wall which was Shotcreted sloughed off and struck the employee's feet. Slurry located behind the metal plate also struck the employee's feet when it sloughed from underneath the plate. This condition exposed employees to struck-by hazards.
