29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  1270 N. Price Road, Olivette, MO 63132: Employer is exposing employees to hazardous conditions where the employees are permitted to work where there are unmounted portable fire extinguishers.  On and about 8/18/2021, portable fire extinguishers were not mounted and readily accessible. Employees were exposed to hazardous tripping conditions where portable fire extinguishers at locations such as, but not limited to, 8C, 7D, and 2F were left on workplace floor, unmounted.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  1270 N. Price Road, Olivette, MO 63132: Employer is exposing employees to hazardous conditions where the employees who are not trained are permitted to operate a forklift. About 08/18/2021, employees who had not been provided required training  operated a Toyota forklift.
